CST Virtual Class Evaluation - May 22, 2021

This form provides the virtual attendee a way to complete the mandatory evaluation of their experience with Zander Perioperative Education, LLC. We need these evaluations to both comply with governing agencies, as well as to continually improve our courses for the best possible user experience.

  • Will the information you gained from attending this program change your practice? Please explain:
  • Do we have your permission to quote your comments on our site - while obscuring your identity by citing only first name, last initial?
  • Once you have filled out and submitted this Evaluation a notification will be sent to our administrator. Soon after, your certificate (emailed as a .pdf to the email you used when you registered) will be sent to you. It will also then be available for you to download from your account page. Please remember we consider ourselves your personal coach, and do not hesitate to get in touch with any question leading up to your exam.
